Special Programs

SEMO Health Network is proud to participate in special programs which serve targeted populations. These include the following:

  • Show Me Healthy Women, a program developed to provide mammograms and annual exams for women over 40 either at a reduced cost or no cost for women who qualify.
  • Chronic Care Collaborative, which tracks and assists in managing diabetes and cardiovascular conditions. Historically, Southeast Missouri has a higher rate of these diseases than elsewhere within the state.
  • Patient Centered Medical Home — SEMO Health Network has been nationally recognized as a Level III Patient-Centered Medical Home (PCMH).  A PCMH is a team-based model of care led by a personal physician who provides continuous and coordinated care throughout a patient’s lifetime to maximize health outcomes.  The PCMH practice is responsible for providing for all of a patient’s health care needs or properly arranging care with other qualified professionals.
  • Medical Home Collaborative — SEMO Health Network has been asked to participate in the Missouri Medical Home Collaborative which is an initiative jointly sponsored by the Missouri Foundation for Health and the Health Care Foundation of Greater Kansas City. The overall goal is to positively impact the health and well-being of Missourians by improving patient-specific and population-based quality of care, reducing costs, and improving patient and care team experience with the healthcare delivery system.
    This will be done through using the patient centered medical home as the model for practice transformation. The Collaborative will support this transformation through technical assistance and a team-based learning experience. The knowledge management portal is a vehicle for sharing information among participating practices in the spirit of “all teach; all learn”.

 

We also take an active role in outreach to the area we serve. Outreach activities include head start screenings, blood pressure screenings at area businesses, dental screenings, migrant registration and health fairs, among many other activities within the area.
