SEMO Health Network collaborates with Missouri Primary Care in the Chronic Disease Collaborative. As a result of our collaboration we are required to meet certain measures and goals.
Diabetic patients with a Hemoglobin A1C less than 7.0 are just one of the many Diabetic Measures we monitor and report on.
Our goal for diabetic patients with a Hemoglobin A1C less than 7.0 is 40% or greater. Below you find the required goal along with a graph showing our actual percentage for SEMO Health Network as an organization as a whole.
Our current percentage of Diabetic patients with an A1C less than 7.0 is 39%. This report is run every 3 months for our Quality Assurance Committee to review, discuss any barriers and plan action steps to help achieve our goal.
